Upload your vaccination verification for a chance to win a Rams Training Camp VIP experience

Dear colleagues,

As an incentive to encourage faculty and staff to upload their vaccination record early, those who do so by August 2 will be entered into a random drawing to win exclusive access to the campus-sponsored Los Angeles Rams Training Camp VIP hospitality tent and reception on August 9. Staff winners will be given release time beginning at 2 p.m. The gates open at 2:30 p.m. and the VIP tent opens at 3 p.m. Those selected will receive two passes to this event.

Employees who previously uploaded their vaccination record, including those who were vaccinated through a UCI employee-only vaccination event, and those who request an exception or deferral by August 2 will be entered to win.

As mentioned in the campus message on July 16, all UCI students, faculty and staff are required to complete their COVID-19 vaccinations two weeks prior to physically accessing university locations and programs. The UC policy requires individuals to share proof of vaccination or receive an approved exception or deferral before returning to campus.

HOW TO UPLOAD COVID-19 VACCINATION RECORDS

Please complete the following steps:

  • Login to the vaccination verification system: https://apps.health.uci.edu/OccHealthRegistrationCampus
  • You will be prompted to upload an image or document of your CDC COVID-19 vaccination card or any other form of vaccination evidence you received from your vaccination provider.
  • You will also be prompted to input basic information such as date of vaccination(s), manufacturer and lot number(s).

CAMPUS STUDENT EMPLOYEES: Your vaccination documentation should be uploaded to the UCI Student Health Patient Portal located on the Student Health Center website, which also includes a video on how to upload your vaccination record on the homepage. You do not need to upload your documentation to the employee vaccination verification system.

HOW TO REQUEST A COVID-19 VACCINATION EXCEPTION

Medical, disability or religious vaccination exceptions and pregnancy deferrals can be requested through the Human Resources Future of Work webpage.
