Dear colleagues,

As we work together toward a bright new day at UCI, including the reopening of our campus in the fall, we ask those of you who have been vaccinated against COVID-19 to please self-identify and upload proof of vaccination.

Employees who received both doses of their vaccination (or one dose of Johnson & Johnson) through a UCI employee-only vaccination event, either at the Bren Center or at UCI Medical Center, do not need to take further action; your vaccination information should already be stored in the employee vaccination verification system.

UPLOAD, VIEW AND PRINT COVID-19 VACCINATION RECORDS

Please complete the following steps:

  • Login to the vaccination verification system: https://apps.health.uci.edu/OccHealthRegistrationCampus/RegistrationV2
  • You will be prompted to upload an image or document of your CDC COVID-19 vaccination card or any other form of vaccination evidence you received from your vaccination provider.
  • You will also be prompted to input basic information such as date of vaccination(s), manufacturer, and lot number(s).
  • If you received your COVID-19 vaccination through a UCI employee-only vaccination event, there will be an option for you to view and print a copy of your vaccination evidence.

When you login to upload your vaccination information, you will see a Privacy Notice that will describe how UCI will use and manage your information.

CAMPUS STUDENT EMPLOYEES: Your vaccination documentation should be uploaded to the UCI Student Health Patient Portal located on the Student Health Center Website, which also includes a video on how to upload your vaccination record on the home page. You do not need to upload your documentation to the employee vaccination verification system.
